Mental Health - Continued Care Service - Outreach Clinic for the Cloncurry Community.  

CCS offer comprehensive care coordination and medical review services in Cloncurry. Our team conducts thorough psychiatric reviews for current consumers, working closely with families and support providers to ensure seamless care. We collaborate with Cloncurry Community Health to provide shared care services and coordinate with both Cloncurry Hospital and Community Health for medication supply and administration. Additionally, we develop culturally sensitive care plans in partnership with Indigenous Liaison staff and facilitate effective discharge planning to ensure smooth transitions for all involved.
